Local non-medical caregiving and PocketRN nurse-led guidance can complement one another for qualifying families.
FAIR OAKS, Calif. - Californer -- Get Care at Home has established a partnership with PocketRN as part of its effort to connect Sacramento-area families affected by dementia with practical in-home support. The relationship brings attention to an important need: caregivers may require both knowledgeable guidance and reliable help with the daily responsibilities of caring for a loved one.
PocketRN provides nurse-led dementia support through the Medicare Guiding an Improved Dementia Experience (GUIDE) Model for eligible people living with dementia and their care partners. Its approach includes education, navigation, and ongoing access to a care team. Get Care at Home contributes non-medical assistance, including help with daily routines and relief for family caregivers, when services are arranged and authorised through the appropriate program.
Dementia care is rarely a single-service problem. A family may need help understanding a new behavior, planning around changing abilities, and making sure someone dependable is present at home. Combining access to guidance with support for daily living can make these arrangements easier to manage.
