Collaboration pairs access to dementia care navigation with local non-medical caregiving resources for qualifying Sacramento-area families.
FAIR OAKS, Calif. - Californer -- Get Care at Home is partnering with Tembo Health to help connect families affected by dementia with appropriate support in the Sacramento area. The relationship aligns two distinct forms of assistance: Tembo Health's dementia care navigation and clinical program resources, and Get Care at Home's non-medical support for everyday life at home.
Through its participation in the Medicare GUIDE Model, Tembo Health describes services designed to help eligible people living with dementia and their caregivers navigate diagnosis, care planning, education, and local resources. Families may also have access to respite support when they meet program requirements. Get Care at Home's role is limited to the non-medical services it is authorised and equipped to provide.
Caregivers often have to make difficult decisions with limited time and incomplete information. Coordination between care navigation and practical home support can help families understand their options and plan around daily routines, caregiver respite, and changes in the person's needs.

Eligibility for Tembo Health's GUIDE services is determined under the applicable program requirements. No family is automatically entitled to a specified amount of respite care solely because of the partnership. Families can learn about available services through Tembo Health and contact Get Care at Home for information on its in-home caregiving services.
