Project with Eviset is designed to improve CalAIM readiness, identify care gaps, and prepare managed care partnership materials.
CITRUS HEIGHTS, Calif. - Californer -- FAIR OAKS, Calif. Get Care at Home has signed a $76,448 scope of work for a CalAIM Providing Access and Transforming Health (PATH) Technical Assistance Marketplace project focused on cross-sector partnership development. The agreement identifies Eviset, Inc. as the technical assistance vendor and sets out work intended to strengthen the agency's readiness to participate in Enhanced Care Management (ECM) and Community Supports under CalAIM.
The planned assistance responds to a practical need: families managing complex disabilities, cognitive conditions, and other care barriers often navigate services delivered by different organizations. Get Care at Home's project focuses on understanding referral pathways, service expectations, and partnership arrangements that can support more coordinated community-based care.
The signed scope includes three deliverables: $17,757 for CalAIM education and staff reference materials; $33,195 for an assessment of Sacramento County providers, care gaps, and partnership opportunities; and $25,496 for managed care plan engagement and application materials. The work places particular attention on people with intellectual and developmental disabilities and related cognitive needs.

The agreement identifies a project period ending October 30, 2026. This is funding for technical assistance services delivered by the approved vendor, not an unrestricted cash grant to Get Care at Home. ECM and Community Supports remain areas of organizational development; the agreement does not itself authorise the agency to furnish those services.
