Sacramento-area care collaboration helps eligible participants receive practical, non-medical support where they live.
FAIR OAKS, Calif. - Californer -- Get Care at Home is working with InnovAge as a home care service partner, providing non-medical support for eligible Program of All-Inclusive Care for the Elderly (PACE) participants in the Sacramento area. The relationship connects the agency's caregivers with a care model intended to help older adults remain in their communities while receiving coordinated support.
Many older adults need help that is neither a hospital service nor a medical procedure. Support with dressing, bathing, meals, familiar routines, and companionship can help make daily life more manageable. Get Care at Home's role is to deliver assigned, non-medical caregiver services according to the care arrangements and authorizations established through InnovAge.
The agency focuses on dependable scheduling, communication with families and care coordinators, and consistent support, including for clients living with memory changes. That emphasis reflects a common reality for older adults and their caregivers: reliable help at home must work alongside a broader care plan.
