SAN DIEGO - Californer -- Serelora, the AI-native agentic EHR, today announced that Serelora Provider, its ambient documentation app, is available on the Apple App Store. The app lets physicians and clinical staff record a patient encounter from their phone and receive a complete, coded clinical note written directly into the patient's chart. Most ambient scribes on the market today generate a note in a separate tool that a clinician then has to copy and paste into the EHR by hand. Serelora Provider removes that step entirely, because the note is written into the same record the practice already runs on.

The workflow was built around how care actually happens. A provider opens the app, selects the patient, confirms consent, and taps record. From there the system listens to the encounter, drafts a structured note organized by chief complaint, history, exam, assessment, and plan, and posts it into the EHR ready for review and signature. Every clinical statement in the note carries provenance back to the moment in the conversation it came from, so a provider reviewing the chart can see exactly where a finding, a medication change, or a diagnosis originated rather than trusting an unlinked summary.

Most ambient scribing tools were built assuming a clinician sits at a laptop, but many practices still run on desktops paired with older monitors that have no reliable microphone and an unstable Bluetooth connection. Every clinician already carries a microphone in their pocket, so Serelora built the capture layer for the provider's phone rather than waiting on hardware the practice was never going to replace.

Serelora Provider is the first release in a broader mobile strategy. The roadmap extends the same on-the-go model beyond capture, allowing providers to issue orders, send referrals, and answer patient messages between rooms, with that work waiting for a final review and signature rather than accumulating until the end of the day. A companion patient app is scheduled to ship next month, giving patients access to the same visit record their provider just signed and an AI they can ask to explain it in plain language.
