Focus on diabetes management, smoking cessation, healthy living
FONTANA, Calif. - Californer -- Knowledge and education are empowering tools in health care, a fact that became even more evident to both patients and providers throughout the COVID-19 pandemic.
Cucamonga Valley Medical Group is a believer in the importance of education, whether we are living through a global pandemic or in what once was known as "normal" times. The CVMG Health and Wellness Resource Center provides patients and their families with educational tools to help them make healthy life choices.
Cucamonga Valley Medical Group's skilled and informed educators cover topics such as diabetes management, smoking cessation and health and wellness through the resource center, which features webinars, blogs, videos and other educational offerings.
"It's your life and we want to help you and your family live it well," said Mary Perry, MD, MPH, a Family and Preventative Medicine Physician. "We're focused on our patients and their families."

The Diabetes Self-Management Education* & Support Program
The Centers for Disease Control and Prevention states approximately 1 in 3 American adults (88 million people) have prediabetes, while 34.2 million have diabetes. CVMG offers The Diabetes Self-Management Education* & Support Program to address this alarming trend.
*The American Diabetes Association recognizes this education service as meeting the National Standards for Diabetes Self-Management Education and Support.
"CVMG is proud to offer this program, which will empower patients with the resources and knowledge to make better and more informed decisions regarding their health," said Dr. Rahmi Mowjood, CEO of Cucamonga Valley Medical Group.
Noe Lopez is a Certified Diabetes Care and Education Specialist. who offers the program to adults age 18 and older who are living with diabetes, have been newly diagnosed, have uncontrolled symptoms or want to make changes to reduce or eliminate the need for medication. The program is offered in both English and Spanish.

Freedom from Smoking
CVMG's Freedom from Smoking Clinic is an American Lung Association accredited program intended to help adults age 18 and older to learn the proven skills and techniques to quit smoking.
"Our health education experts will provide the guidance and tools to help people quit smoking," said Dr. Perry.
Healthy Living Program
CVMG's monthly Healthy Living Program focuses on healthy lifestyle choices. A better diet. More exercise. Smart choices.
"Participants will learn more about basic human physiology and receive useful tips and evidence-based advice to make more informed decisions to improve their health," said Sabrina Tupuivao, Community Health Educator at Cucamonga Valley Medical Group.
